+ How do I register my Test?
To register your test, go to www.biotechaccelerated.com/register and submit your order number and sample ID. Then, you will be redirected to Curogram’s site where you will go through the registration process (2-3 minutes). Once you finish Registration, you should receive a text and email confirmation that come with a link to an instructional video that will help guide you through the process of taking your test. You will also receive a check in notification and a Test Requisition Form, which you may use to submit a claim to insurance for reimbursement after receiving your results. Be sure to read the instructions in your kit before taking your test.
+ I’m registering, but the text is too small for me to read. What should I do?
To make the text on your computer larger so you can read it, zoom in.
On a Mac:
- Click "View" on your menu bar. Then click "Zoom in"
On a PC
- Hold down the Windows key and click the plus sign to Zoom in
+ Can I enter my Landline instead of my cell phone number?
In the demographics section of Registration, you may enter your landline if you do not have a cell phone number. You will have to enter your email during the first step of registration instead of a cell phone number so that you can receive notifications.
+ Why do I need to register my Test?
In order to receive your results from the lab, you must register your Test. Each Test Kit must be registered individually with the patient information that matches the person being tested. The information you input during Registration will be used to send you your results. Additionally, state and local governments require our lab to collect demographic information from you in order to provide you with your results. See the CDC website for more information. Finally, in order to qualify for our physician to order your Test, you must be able to answer “Yes” to one of the screening questions on our questionnaire about why you are taking the Test.
+ Can I register multiple people using the same email or phone number?
You can use the same phone number and email to register multiple Tests as long as you add a patient during step 3 of the process. Patient information entered during Registration MUST match the person who is being tested. Patient information cannot be changed after the Test Kit has been returned to the laboratory. If you have made a Registration error, you may reply to your text and email to request that it be corrected.
Each Test must be registered individually by the adult taking the test or, if the test is taken by a minor, their parent or guardian.
+ What is the Lab Requisition Form and why do I need to fill it out?
The Lab Requisition Form is the form inside your kit that says "Lab Requisition Form" on top. You must fill it out with information about the person taking the Test, and this information must match any similar information you input during Registration. You MUST fill out this form in order to receive your results.
+ Is my patient health information protected?
The Health Insurance Portability and Accountability Act (HIPAA) and supplemental legislation collectively referred to as the HIPAA rules (HIPAA) lay out privacy and security standards that protect the confidentiality of protected health information (PHI). The information that you provide to Biotech Accelerated in the course of submitting your test comprises PHI.
In the course of providing services to you and to enable HIPAA compliance to covered entities such as Curogram with respect to the processing of your test submission, Biotech Accelerated complies with the provisions of HIPAA which are required and applicable to Biotech Accelerated in its capacity as a “Business Associate” under HIPAA. + Why do I need to answer questions about why I am taking the Test?
In order to qualify for our physician to order your test, you must answer yes to one of the possible reasons given for taking your Test.
+ How long can I wait to take my Test after registering?
We ask that you do not register your Test until you are ready to take it.
+ What do I do if I made a mistake during Registration but cannot go back and fix it?
If you have made a mistake during registration, use the number you registered with to text 888-414-6199 (or reply to your registration confirmation email) with details about your registration error. You will receive confirmation once your error has been fixed. You may also message Biotech Accelerated using the Curogram App, which you can download here. For questions that don’t concern registration errors, contact customer support at 800-884-2212.
+ What if I can’t find my email or text with the code to verify my email or phone number at the beginning of registration?
If you can’t find your email, try searching “Curogram” in your email search bar for an email with the subject “[Curogram] Message Notification.” Also, check your spam folder, and go back to make sure your email was spelled correctly. If you still can’t find it, try registering with another email.
If you can’t find your texted code, go back and check if your phone number was entered correctly.
+ What happens if I lose the pamphlet, instructions, or Lab Requisition Form in my box?
