+ How do I set up my Kit so that it is ready to be sent to the lab?

  1. DOUBLE CHECK the cap on your specimen tube is tightly closed.
  2. Place specimen tube and completed Lab Requisition Form in biohazard bag and press firmly across the closure to securely zip.
  3. Place the bagged specimen and Lab Form into the box and place the absorbent sheet on top.
  4. Rip off the plastic strip on the box to reveal the seal, and close the lid.
  5. Place your enclosed return label on the box without covering the diamond or serial number.
  6. Write down your return label tracking number to track at ups.com/track
  7. Drop off the sealed return box at the closest UPS Drop Off within 24 hours of specimen collection. Find your location at ups.com/dropoff b)
    • Note: When searching for UPS drop off locations, check the box for “accepts restricted articles” on the ups.com/dropoff online search tool to avoid shipping delays. Do not drop off this test at a UPS drop box on weekends as this will incur delays.

+ Do I need to leave my home to drop off my sample at a UPS Drop Off location?

No. You can also arrange for UPS to pick up your package at your home. However, this option will cost a fee. Explore this option by clicking here.

Also, if UPS is coming by your house to deliver a different package, you may give this one to them while they are already at your house. They will usually accept your package free of charge in this case.

+ What happens if I delay sending back my Test after taking it?

Send your sample to the lab within 24 hours of collection. Do not delay sending your Test back to the lab.

+ Can I drop my Sample off at a UPS drop box?

We discourage dropping your Sample off at a UPS drop box because it may delay your shipment. Make sure you drop it off before the pick up time. Do not drop off this test at a UPS drop box on weekends as this will incur delays.
