+ I want to order a test or tests. What is the process?

To order your test, you must first fill out a health screening form to determine that you qualify for testing. This takes 2-3 minutes and can be completed here. Make sure your name / address / contact info match the information associated with your order. Then call 800-884-2212 or go to ShopHQ.com. + What is the Testing process?

  • SCREENING

    First, you must complete a short screening form to determine if you qualify to order a Test. You must answer YES to at least one of the questions on the screening form to qualify. Your Test Kit(s) cannot be shipped to you until you complete this step, so please do so right as you order. Only one screening is requied per order, even if you order multiple Test Kits. Screening is required so that we can provide you with a Doctor's order for the Kit(s).

  • ORDER

    Next, order your kit. It will arrive within 1 day if you select next day service. If you select ground transportation, it will arrive within 1 to 5 days.

  • REGISTER

    Once your Kit arrives and BEFORE you take your Test, you must register your Sample ID here with the matching Order Number. If you do not register your Test, you will not be able to take access your results. For more detail on registering your Test, click here. Each Test Kit must be registered individually with the patient information that matches the person being tested.

  • COLLECT

    Before you collect your sample, do NOT eat, drink, chew gum or smoke for 30 minutes and read the instructions enclosed in your collection device. Then watch this instructional video, which will help guide you through the process of collection. For more detail on taking your Test, click here.

  • LAB FORM

    Fill out the Lab Requisition Form (found in your box).

  • RETURN

    After collecting your sample, set it up to be returned to the lab along with the Lab Requisition Form. For more detail on this step, click here.

  • RESULTS

    You will then receive your results via text and email (or on your patient portal) within 48 hours from your sample’s arrival at the lab. If you have a positive result or have questions about your result, you may elect to speak to a healthcare professional here. For more detail on Test results, click here.

  • INSURANCE REIMBURSEMENT (Not guaranteed)

    You can submit your Test Kit receipt and your Test Requisition Form (sent when you register) to your insurance for reimbursement (along with the completed claim form your insurance provides). For more information, click here.

+ What do I do if I have trouble with my order?

+ Can I order tests for others? How many tests can I purchase at a time?

Yes, you can purchase tests for yourself and others. You can purchase any number of tests at a time. However, each test must be registered individually by the adult taking the test or, if the test is taken by a minor, their parent or guardian.

+ I need test results by a certain date. When should I order my test to ensure I can get results in time?

Once you order a test and complete the 2-3 minute screening process, it should arrive within 1 business day if you select next day service (may not ship immediately on weekends or holidays). If you select ground transportation it should arrive within 1 to 5 business days. Once you register your test and collect your sample, each test has a pre-paid overnight return UPS label to send your test back to the lab. You will receive your results within 48 hours of the test kit’s arrival at the lab. Thus, if you want your results by a certain date, you should order your test 5-9 business days in advance depending on the shipping option you select to leave room in case of UPS delays.

+ Can I order a test and not take it immediately? How long does the test last?

There is a use by date directly on the tube label. All tests will have a minimum of a 3 month shelf life after they are purchased. Typically, they can be used for up to 24 months (but check your device to make sure).

DO NOT register your kit on our website until you are ready to take the test. We ask that you register the same day you take your test. Once you take your test, drop it off at a UPS Drop Off within 24 hours. Find your nearest UPS Drop Off location here.
